In the figure caption of Fig. 4.11 we see that the paper took almost three years to be published. The reason was: one referee requested further or refined calculation of some problems. It was not so simple to respond the request, and almost half a year was needed. Upon resubmission a new referee showed up, asking another completely different request. This went on three or four times.
